When sending out correspondence related to a registrant's personal data, you need to have set up the contact information for privacy-related correspondence. The privacy contact email is necessary if you wish to use the Personal Data Email feature, to send an individual the personal data collected in the Visit system. It is a setting in your Organisation's profile. For more information, see Sending a Personal Data Email.
- On the Dashboard, select Form > Forms.

Click Add Form.
- Select the form type:

Click Save.
- Enter a Form title.
- Enter a Description (optional).
After creating a form, the first thing you'll want to do is edit the Welcome page. See Edit Welcome Page.
